BHRT or Bio-Identical Hormone Replacement Therapy

BHRT is used to treat both men and women who may suffer from low or imbalanced hormones.  These hormones are molecularly identical to the ones produced in the body. Hormones are special chemicals that control most of your basic bodily functions.  They serve as an internal communication system between cells throughout the body and coordinate everything from digestion and growth to your appetite, immune function, mood, and libido. BHRT may also assist with your muscle mass and bone density, which can help you stay active and independent as you age. Because every patient is different, we use blood tests to analyze your hormone levels and then determine your individual hormone profile.

The main female hormones are estrogen and progesterone which are produced in your ovaries during your menstrual cycle.  Testosterone is also produced in the ovaries, adrenal glands, fat, and skin cells. The female body readily converts testosterone into female sex hormones. During menopause, usually occurring in your late 40’s or early 50’s your ovaries slowly stop producing these hormones leading to unbearable symptoms. A hysterectomy will cause symptoms of menopause.

BHRT for men offers a safe and effective way to improve your testosterone levels with natural hormone replacement therapy.

As men age, their bodies go through a variety of changes. How it’s done:

A panel of blood work will be drawn and analyzed before the initial procedure. These hormones are placed in a tiny pellet, about the size of a grain of rice, and inserted just under the skin in the upper gluteal area during a safe, simple office procedure.  The procedure itself takes about 15 mins.  The pellets will deliver consistent hormone levels over an extended period, usually 3-4 months for females and 4-6 months for men. The pellet slowly dissolves as the body draws small physiologic dosages of the hormone to maintain a more consistent delivery.  Once it is dissolved, the procedure is repeated, and new pellets are inserted. 

Who is not the ideal BHRT candidate?

Known, history, or suspected cancer, high-risk endometrial or breast cancer, current or previous deep venous thrombosis or pulmonary embolism, stroke, TIA, history of blood clotting disorder, active liver disease or unexplained vaginal bleeding.

Semaglutide For Weight Loss

What is Semaglutide?

You may know of it by the brand names Ozempic or Weygovy. It is currently the # 1 weight loss medication available. Studies have shown that on average participants loss an average of 15.8% of their total body weight. Some have seen even more of a weight reduction.  Semaglutide is an injectable medication which was originally and still is used in type 2 diabetes for blood sugar control. It was found that when combined with diet and exercise, these diabetic patients also experienced unexpected weight loss.  Semaglutide has now been approved by the FDA for weight loss treatment in adults with a BMI greater than  30 kg/m², and adults with a BMI greater than 27 kg/m² who have at least one weight-related comorbidity, like high blood pressure or high cholesterol, etc.  Semaglutide belongs to a class of medications called glucagon-like peptide-1 (GLP-1) agonists, which mimic the hormone GLP-1 in your body to lower blood sugar levels after you’ve eaten a meal. Semaglutide is not a type of insulin, instead a hormone that stimulates your pancreas to release insulin when glucose, aka sugar, is present.  GLP-1 inhibits glucagon release from your liver (which slows down the release of sugar into the blood so that you burn more fat), slows down gastric emptying (makes you feel full), and lowers the desire for food intake (because you feel full). The combination of these actions leads to glucose homeostasis which triggers the burning of fat storage for energy.  You should feel less hungry, prompting you to eat less and lose more weight.  

How is it given?

Semaglutide is weekly injection that is given subcutaneously into your belly fat. You are started on a small dose of .25mg and it is increased monthly until you have reached the goal of 2.4 mg once weekly.  It is given in small increments to avoid possible GI side effects. We can give you these weekly injections or teach you how to give them to yourself.

  • Constipation

Who should not take Semaglutide

If you are pregnant, trying to become pregnant, lactating, a type 1 diabetic, have a history of medullary cancer, thyroid cancer, endocrine neoplasia syndrome, pancreatitis, liver disease, kidney problems or already taking medications which reduces your blood sugar.

Is Semaglutide safe?

Yes. Semaglutide is considered to be safe and effective when used as indicated. But safe doesn’t mean that there aren’t risks. Semaglutide also carries a boxed warning about thyroid C-cell tumors occurring in rodents (with unknown risk in humans), and Semaglutide shouldn’t be used if you or your family have a history of certain thyroid cancers. Semaglutide should not be used in people with type-1 diabetes or a history of pancreatitis. Semaglutide should be used cautiously for people on other blood sugar lowering medications.

Is Semaglutide covered by my insurance?

Unfortunately, Semaglutide is not covered by insurance for people who are not type 2 diabetics.  However, you can get this medication as part of our Semaglutide Weight loss program, and it is affordable. We use a compounding pharmacy which cuts the cost down to 1/3 of the price.

PRP for Joint Pain

What is PRP?

PRP (platelet rich plasma) therapy for joint pain is a three-step medical treatment in which a person’s blood is drawn, processed, and then injected into the affected area. Although blood is mainly a liquid (plasma), it also contains small solid components (red cells, white cells, and platelets). The platelets are best known for their role in clotting blood; however, platelets also contain hundreds of proteins called growth factors that are very important in the healing of injuries.  Platelet activation triggers the body’s natural healing process, cell reproduction and stimulates tissue regeneration. It uses your own blood particles to heal and improve musculoskeletal problems.  After the PRP is separated from the whole blood, this rich plasma will contain many more platelets than what is typically found in blood. The concentration of platelets and growth factors can be 5 to 10 times greater (or richer) than usual which will seal, and fill injured or damaged tissue.

Can I benefit from PRP joint injections?

Recovery period:

The actual procedure is done using guided imaging to ensure that the PRP is placed precisely in the intended area.  It takes about 30 minutes in the office to complete and there is little downtime, and you can drive yourself home afterwards. Exercises and NSAIDs can be slowly reintroduced after one week. Steroids should be avoided for 4 weeks.  Over the next 2-3 months, pain should be decreasing while the function of the joint should continue to improve.

Who would not make a good candidate for musculoskeletal PRP:

Pregnancy, breastfeeding, platelet abnormalities, anticoagulation therapy, severe liver disease, current or previous cancer, anemia, excessive smoking, or drinking.

XEOMIN

Closely related to Botox, Xeomin is formulated to be purely botulinum Type A.  Xeomin is also known as the “naked” injectable as this formulation reduces the chances of allergies.  Xeomin is used to soften minor to moderate wrinkles in the forehead and near the eyebrows and eyes. This purified protein is injected into facial muscles to combat and revitalize lines that occur with aging.

You should not get Xeomin if you are allergic to any of its ingredients, had an allergic reaction to any other botulinum toxin product, pregnant, planning pregnancy, breastfeeding, nerve or muscular issues, or a local skin infection at planned injection site.
